Determinants of never having tested for HIV among MSM in the Netherlands

Abstract

Objectives: Men who have sex with men (MSM) who are unaware of their HIV infection are more likely to infect others, and unable to receive treatment. Therefore, we aimed to identify the proportion and characteristics of Dutch MSM who never tested for HIV.

Methods: In 2010, the European MSM Internet Survey (EMIS) recruited 174,209 men from 38 countries through an anonymous online questionnaire in 25 languages. We analysed data from participants living in the Netherlands (N=3787). The outcome we investigated was having never (lifetime) been tested for HIV.

Results: A total of 770 MSM (20.4%) had never been tested for HIV. In multivariate regression analyses, not being from Amsterdam (adjusted OR, aOR 1.54, CI 1.17 to 2.03), with low education (aOR 1.28, CI 1.04 to 1.57) and low knowledge on HIV-testing (aOR 2.23, CI 1.37 to 3.64) were significantly associated with never having tested. Lower sexual risk (including having fewer sexual partners (aOR 2.19, CI 1.57 to 3.04) and no anal intercourse (aOR 5.99, CI 3.04 to 11.77)), and less social engagement (including being less out (aOR 1.93, CI 1.55 to 2.40)) were also associated with having never been tested. Additionally, 36.1% of MSM who never tested for HIV reported high-risk sexual behaviour that may have put them at HIV risk.

Conclusions: MSM make their own risk assessments that inform their choices about HIV-testing. Nevertheless, MSM who were never tested may have been at risk for HIV, and remain important to target for HIV interventions.

Abstract

The group of men who have sex with men (MSM), is the most affected by HIV, both in Western and Central European countries and in our country. Therefore, the European Commission has funded internet study of MSM community in 33 countries, covering the period from April 2009 to September 2011. The respondents from Bulgaria are 1036. The results of the study in our country showed that more than half of the involved subjects (52.3%) identified themselves as gay or homosexual, one in four (27.7%) considered themselves as bisexual, and 17.1% do not use any term to define their sexual behaviour. Those who claim to know their HIV status are only 68.5% of all respondents; the remaining 31.2% are not sure. It is a concern that a significant proportion (61.4%) of respondents who did not know their HIV status and are sure that they are HIV negative. The information from the survey could be used as a corrective to the already taken initiatives and as a supplement to the new prevention strategies when planning new activities for reducing the spread of HIV/AIDS in Bulgaria at national and regional level. EMIS-2010 National reports

EMIS national reports are provided by Associated and Collaborating Partners of EMIS. They are available in the language(s) of the country of the repective partners and are based on the data provided by respondents who lived in that country at the time of the survey (irrespective of the language they used to fill in the questionnaire).

No national report will be published for eight countries because of small sample sizes: Belarus, Bosnia & Herzegovina, Cyprus, Croatia, Luxembourg, Macedonia, Malta, and Moldova. No national reports were planned for: The Netherlands, Finland, Hungary, Romania, Serbia, and Ukraine. National Reports had been announced for Bulgaria, for Czechia and Slovakia, France, Greece, Italy, Lithuania, Slovenia, and Turkey, but were never published.

In the UK, local area data reports were made available March 2011, giving HIV prevention comissioners and planners access to detailed EMIS data about their local residents. EMIS summary reports for England, Scotland, Wales, and Northern Ireland.

European report 2010

The EMIS final report was written by the Associated Partners of EMIS between July and September 2011, and includes a chapter written by ECDC. It is called  “EMIS 2010: The European Men-Who-Have-Sex-With-Men Internet Survey. Findings from 38 countries” (ISBN 978-92-9193-458-4). 

Please note that the pdf below has been updated (new version 1.5) on 20 March 2014. It is also available to download also on the ECDC website. Print copies (version 1.4) are avaiable free of charge via the EU Bookshop.
